Responsibilities include identifying “at risk”

identified persons; developing specific care plans,
strategies and processes for these clients. Maintain a good
understanding of the disease(s) process and management to
optimize a client’s compliance. Maintain consistent
contact with clients and evaluate changes in physical,
mental and/or social conditions. Respond to daily
utilization reports (hospital, emergency room). Develop,
implement and maintain a client tracking and monitoring
process that will assist the client in accessing appropriate
health and support services. As job duties require travel to
homes of patients and visits to physician offices, you will
