Opportunities Administrative Case Manager PRIMARY PURPOSE OF
DEPARTMENT: The primary function of the Administrative Case
Manager is to serve consumers by ensuring appropriate
treatment, case linking, and referral to other resources and
to act as a liaison to community facilities. This is done in
accordance with the Agency’s Vision, Mission and Values.
PRIMARY PURPOSE OF THIS POSITION: The primary purpose of
this position is to function as a team member with Service
Coordinating Unit, other programs within the agency,
community and institutional resources. Utilization of these
services will present the opportunity for consumers to move
towards a more stable, meaningful and productive life.
DESCRIPTION OF DUTIES: - Contact and schedule an evaluation
with Administrative Case Management consumer referrals
within Emergent, Urgent and Routine access standards.
Develop and implement a plan of action, making
recommendations and linking to appropriate levels of
